WB SIR 2025: A Complete Guide to Filling Out the Offline Form for Voter List Revision

WB SIR 2025: The process of updating the voter list has begun across India, including West Bengal. The Election Commission has released an offline form for new voter registration, correction of information, and change of address. While the process of filling out this form is straightforward, problems can arise if some information is not entered correctly. Where to get the form?

The first question that may come to your mind is where to get this form. You can get this form from the Booth Level Officer (BLO) of your area. He will also give you the necessary instructions for filling out the form. Although there is a provision to fill this form online, that process has not yet started in West Bengal. So for now, the form has to be filled out offline.

Important things to know before filling out the form

Some important things need to be kept in mind while filling out the form:

  • Pre-printed information: Some parts of the form may have your name, EPIC number, address, voter list serial number, part number, booth name, assembly and parliamentary constituency names, and the name of the state pre-printed. If this information is not pre-printed, you will have to fill it in.
  • Photo: An old photo of you will be printed on the form. You have to paste a new passport-size photo over it.
  • Mandatory and Optional Information: Some information on the form is mandatory and some is optional. Let’s see which are mandatory and which are optional:
    • Date of Birth: This must be filled in.
    • Aadhar Number: This is optional.
    • Mobile Number: This is mandatory.
    • Father’s/Legal Guardian’s Name: This is mandatory. Their EPIC number is optional (“if available”).
    • Mother’s Name: This is mandatory. Her EPIC number is optional.
    • Spouse’s Name: Applicable only for married individuals. Their EPIC number is also optional (“if available”).

Declaration and Signature

At the end of the form, there is a declaration section. You have to tick a box there. Then you have to mention the date and place (your home address and the date of filling the form). Finally, you have to sign or provide a left thumb impression.

Some important advice

There is no need to panic while filling out the form. If you do not have optional information like an Aadhar number or EPIC number, there is nothing to worry about. You can leave that space blank. No formal notification has yet been issued by the Government of West Bengal on this matter. We will provide you with more detailed information once the notification is issued.
