SIR Form Fill Up: The Election Commission has introduced new rules for the revision of voter lists, leading to some changes in the process of filling up the SIR (Summary Intensive Revision) form. Your local BLO (Booth Level Officer) will deliver this form to your home. It is crucial to fill out this form accurately. Let’s understand the complete process step-by-step according to the new guidelines.
The upper part of the form will come pre-filled with your details such as your name, EPIC card number, and address (as per the 2025 voter list). Your serial number, Lok Sabha, and Assembly constituency names will also be mentioned.
New Guidelines Regarding Photographs
The form will have a space to affix a new passport-sized photograph next to your current voter card photo. However, affixing a photograph is not mandatory. Sometimes this space can be very small.
- Photo Size & Background: There is no strict rule that the photo must be 3.5cm x 3.5cm or have a white background. Your form will not be rejected for this.
- What if You Don’t Provide a Photo?: Even if you do not paste a photo, your name will still appear on the SIR draft list.
- Dark Background: Photos with a dark or coloured background might cause minor difficulties for the BLO during the app upload, but it won’t lead to form rejection.
Filling the Upper Part of the Form (As per 2025)
In this section, you need to fill in some personal details. The information provided here will be applicable for the 2025 voter list.

Join on Telegram- Date of Birth: If only the year of birth is mentioned on your voter card, you can write the full date of birth as per your Aadhaar card. However, this will not update your new voter card. For updates, you must fill out Form 8.
- Aadhaar Number: This is optional. If you choose to provide it, ensure the number is correct, as the BLO app will flag incorrect entries.
- Phone Number: Provide a correct and active phone number.
- Father’s or Guardian’s Name: Everyone, including married women, should write their father’s name here. Only orphans or those with a legal guardian should write the guardian’s name.
- Spouse’s Name: There is a separate field (Spouse Name) for married women to write their husband’s name. Do not write the husband’s name in the guardian’s column.
- EPIC Numbers of Relatives: Provide the EPIC card numbers of living relatives only (father, mother, or spouse). If a relative is deceased, leave their EPIC number field blank. Do not write ‘Late’ or any similar prefix before the name of a deceased person; just write the name.
Filling the Lower Part of the Form (As per the 2002 List)
This section has two parts. The box on the left is for those whose names were on the 2002 voter list.
First, you need to download the 2002 electoral roll and find your name or a family member’s name in it. Then, fill in the following details accordingly:
- Elector’s Name: Write the name exactly as it appears on the list.
- EPIC Number: If an EPIC number is mentioned in the list, write it down. If not, leave the field blank.
- Relative’s Name: Write the relative’s name as mentioned in the list.
- Relation: Mention the relationship (e.g., Father) as specified in the list.
- District, State, Assembly: Write the name of the undivided district, state, and assembly constituency where you were a voter in 2002. This information is available on every page of the list.
- Assembly No., Part No., Serial No.: You will find all this information next to your name in the 2002 list.
