Download New PAN Card Form PDF: Starting April 1, 2026, the rules for applying and correcting PAN cards for Indian citizens will undergo a massive transformation. Instead of the familiar old forms, completely new and specific forms will be introduced. This change aims to simplify the process for citizens while making document verification more stringent.
Major Rule Shifts and Public Impact
The most significant update in the new guidelines concerns the proof of date of birth. Previously, an Aadhaar card alone was sufficient. However, under the new rules, submitting an additional age proof like a Madhyamik admit card, passport, or birth certificate alongside Aadhaar is mandatory. To eliminate confusion regarding names, applicants will now directly enter their First Name followed by the Last Name. Another notable addition is the ‘Single Father’ option, ensuring inclusivity that was missing in earlier forms.
Total Revamp of Application Forms
The old 49A and CSF forms will soon be history. For better categorization, the authorities have introduced different forms for different types of applicants.
- Individual Applications: General citizens will now have to use ‘Form 93’.
- Commercial Entities: Shops and business organizations will use ‘Form 94’.
- Foreign Nationals: Non-residents or foreigners are required to fill out ‘Form 95’.

A Closer Look at the Three-Page Form
The new form for individuals is more detailed and has been expanded to three pages.
- First Page: Applicants must affix two specific-sized photos. The signature spot has shifted to the left, requiring individuals to sign across the photo. This page captures personal details, address, and contact information based on Aadhaar.
- Second Page: This section focuses on parental details, offering the choice to print either the father’s or mother’s name on the card. For minors, the Representative Assessee’s details are mandatory. Options to provide a Passport or TIN number are also included.
- Third Page: The final page is dedicated entirely to the declaration. The applicant or their legal guardian must sign here.
The Revised Correction Process
The method for rectifying errors in an existing PAN card has also been streamlined. Applicants must first write their current PAN and Aadhaar numbers. After that, they need to tick the specific checkbox (name, gender, DOB, or address) that requires an update. A major benefit of this revised correction form is the new facility to link a Passport or TIN number directly to the PAN card.
