SIR Enumeration Form: The Booth Level Officer (B.L.O.) might have already visited your home with the SIR (Special Summary Revision) Enumeration Form. It is crucial to fill out this form correctly, as even a minor mistake could lead to the rejection of your application. Understanding this new process by the Election Commission is essential. Let’s break down each step of filling out this form in a simple way.

The entire form is primarily divided into three main steps that every voter needs to complete.

Step 1: Preliminary Details

At the very top of the form, there is a space for your name and address, although this section is mainly for the B.L.O.’s use. Alongside this, you will find a specific serial number, part number, and a unique QR Code assigned to you.

  • Old Photograph: There is a designated space to affix your old photograph from your previous voter ID.
  • Recent Photograph: Next to it, you need to paste a recent or current photograph of yourself.

Step 2: Key Personal Information

In this section, you must carefully fill in some personal details.

  • Date of Birth: Enter your correct date of birth.
  • Aadhaar Number: Providing this is not mandatory; it is optional.
  • Mobile Number: Mention your active mobile number here.
  • Father’s and Mother’s Name: First, provide your father’s name along with his voter card’s EPIC number. Similarly, you must also mention your mother’s name and her EPIC number.
  • Husband’s Name: If an applicant’s parents are not alive, an alternative has been provided, especially for women, to give their husband’s name. If the husband has a voter card, his EPIC number should also be mentioned.

Step 3: Linking with the 2002 Electoral Roll

This is the final and most critical stage of the form, which is divided into two parts (left and right).

Left Side of the Form:
In this part, you need to mention the name of an elector who was on the revised list of 2002 and state your relationship with them.

  1. First, write your own name and EPIC number.
  2. Next, provide the name of your relative whose name was on the 2002 list. According to sources, this relative can be your father, mother, or someone from your paternal grandfather’s side.
  3. Specify your relationship with them (e.g., Father, Mother).
  4. Mention that person’s district, assembly constituency, and the part number of that constituency.

Right Side of the Form:
If your name was not on the 2002 revised list and you have mentioned a relative (father/mother) on the left side of the form, then you must fill in all the details of that relative (name, EPIC number, relationship, etc.) again in this right-hand section.

For What Reasons Can the Form Be Rejected?

The Commission has clearly stated that your form can be rejected for two main reasons:

  • If you are not a citizen of India.
  • If any information provided by you is proven to be false or fabricated.

In such cases, not only will the form be rejected, but the Commission may also take appropriate legal action against you. Therefore, if you have any doubts while filling out the form, seek assistance from the on-duty B.L.O. immediately.
