The correct procedure for filling the form depends entirely on whether the woman’s name was present in the voter list of 2002.

Scenario 1: Name Was NOT on the 2002 Voter List

For married women whose names were not on the 2002 voter list, but a parent or another relative was listed, the right-hand column of the form must be completed.

  • Primary Information: At the top of the form, fill in the woman’s full name, current EPIC number, address, and affix a new colour photograph. Also, provide your date of birth, Aadhaar number (it’s optional but highly recommended), and phone number.
  • Guardian’s Details (as per 2025):
    • Father’s or Guardian’s Name: In this section, married women must write their father’s name.
    • Mother’s Name: Mention your mother’s name and her EPIC number, if available.
    • Husband’s Name: Write your husband’s name and his current EPIC number, if available. If the father, mother, or husband is deceased, only their name is required; the EPIC number is not necessary.
  • Relative’s Details (as per 2002):
    • This section requires the name of the relative based on whose details you are filling the form (e.g., father, mother, grandfather).
    • Next, enter that relative’s EPIC number, name as mentioned in the 2002 voter list, and your relationship with them. It is crucial to copy the details exactly as they appear in the 2002 list to avoid discrepancies.
    • Finally, fill in the district, assembly constituency, part number, and serial number as per the 2002 list.

Scenario 2: Name Was ON the 2002 Voter List

Married women whose names were already included in the 2002 voter list need to fill out the left-hand column of the form.

  • Primary Information: The rules for the top section remain the same. Fill in your current name, EPIC number, address, photo, date of birth, Aadhaar, and phone number.
  • Guardian’s Details (as per 2025): Here too, in the ‘Father’s or Guardian’s Name’ field, you must write your father’s name. Following that, provide your mother’s and husband’s names along with their current EPIC numbers, if available.
  • Your Own Details (as per 2002):
    • Elector’s Name: You must write the name that was on the 2002 voter list, which is likely your maiden name. For example, if your current name is Ruby Dutta, but your name in 2002 was Ruby Mukherjee, you must write Ruby Mukherjee.
    • EPIC Number: Write your EPIC number as per the 2002 list. If the number is not mentioned in that list, you can leave the space blank.
    • Relative’s Name & Relation: Mention the relative’s name (e.g., your father’s name) and the relationship exactly as it was recorded next to your name in the 2002 list. Even if there are spelling mistakes in the old list, copy them as they are.
    • Finally, provide the district, assembly constituency, part number, and serial number from the 2002 voter list.

Remember to fill out the form in Bengali, but numbers like EPIC, Aadhaar, and phone numbers should be written in English. Lastly, sign in the designated space. If you have any further doubts, it is always best to consult with your local Booth Level Officer (BLO) to prevent any errors.
