SIR Form Fill Up: The process of filling out the enumeration form for the West Bengal voter list, popularly known as the SIR Form, has commenced. While this form, issued by the Election Commission of India, is not overly complicated, it requires careful attention to specific details. A minor error can create problems in your application process. This report provides a detailed, step-by-step guide to filling out the form accurately.

First Part of the Form & Pre-Printed Details

When you receive the form, you will notice that the first section already contains some of your information pre-printed. You do not need to fill anything in this section; simply verify the details against your current voter ID card.

  • BLO’s Information: The name and phone number of the distributing Booth Level Officer (BLO) will be written in English at the top of the form. You can contact this number if you have any questions or face any issues.
  • Voter’s Details: This section will have your name (in Bengali), EPIC Number, address, serial number, part number, assembly constituency, and state name pre-printed. A QR code and the photograph from your current voter card will also be present.

Voter’s Current Information and Photograph

In this section, you need to fill in some information and affix a photograph.

  • Recent Photograph: Paste a recent colour passport-size photograph in the designated space. It is advisable for the photo to have a white or light-coloured background, as online forms often require this.
  • Date of Birth: Write your date of birth in English in the DD-MM-YYYY format.
  • Aadhaar Number: Although optional, it is highly recommended to provide your Aadhaar number for accuracy. Write it in English.
  • Phone Number: Enter your current mobile number in English.
  • Father’s/Guardian’s & Mother’s Name: Mention your father’s name. The term ‘Guardian’ is applicable only for residents of orphanages. Married women should also write their father’s name, not their husband’s. If your father or mother has a current EPIC number (as of 2025), mention it.
  • Spouse’s Name: This section is to be filled only by married individuals. Mention your spouse’s name and their current EPIC number (if available). This part should be left blank if you are unmarried, widowed, or divorced.

This is the most crucial part of the form and must be filled based on the 2002 voter list. It is divided into two parts.

A. For those whose names were on the 2002 voter list:
Individuals who were on the 2002 electoral roll should fill this section. You need to enter your name as it appeared in the 2002 list (even if the spelling has changed since), EPIC number (if any), relative’s name and relationship, and address details (District, Assembly, Part No., Serial No.) as per the 2002 list.

B. For those applying based on a predecessor’s details:
Those who became voters after 2002 can fill this section using the 2002 voter list details of their father/mother/grandfather/grandmother. Here, you need to provide the relative’s name, their 2002 EPIC number, the relative’s name as mentioned on their card, and their address details from that list. The relationship of the applicant with that relative (e.g., Father, Mother) must be mentioned.

Important Instructions Before Submission

  • Signature: Sign or provide your left thumb impression in the designated space. If you use a thumb impression, a witness must also sign.
  • Documents: No additional documents need to be submitted with this form.
  • For Married Women: Married women must use the details of a blood relative (father, mother, grandfather, or grandmother). Even if your surname has changed after marriage, you must write your pre-marriage name and surname as it was in the 2002 voter list.

To avoid errors, do not rush. It is a good practice to first fill out a photocopy of the form and then fill in the original copy.
