How Married Women Should Fill Up SIR Form Check Correct Rules and Method Based on 2002 Data

SIR Form: Filling out the SIR (Supplementary Information Register) form is a crucial process for verifying voter list data and maintaining accuracy in West Bengal. For married women, this process can sometimes be confusing due to changes in surnames and addresses after marriage. This article details the correct method for married women to fill out the SIR form accurately based on the provided guidelines. Below is a step-by-step guide on how to fill the form based on the 2002 voter list data.

General Rules for Filling the Form (Applicable to All)

The upper section of the form needs to be filled out by every applicant using current (2025) information.

  • Name and Address: Write your name, EPIC number, and address exactly as they appear on your current voter card.
  • Photo: Affix the photo from your current voter card and a recent passport-sized photograph in the designated space.
  • Date of Birth: Provide your correct date of birth including day, month, and year. Even if your voter card only shows the year or ‘XX’, you must write the full correct date on the form.
  • Father’s or Guardian’s Name: For married women, even if the husband’s name is on the voter card, it is preferable to write the Father’s Name on this form to establish lineage. However, if there is a specific separate column for the husband’s name, you may write it there.
  • Other Details: Provide your Aadhaar number (optional but recommended) and an active mobile number. Fill in details of family members like father, mother, and husband along with their EPIC numbers if they are in the 2025 voter list. If any member is deceased, only write their name.

Situation 1: Name NOT in the 2002 Voter List

Applicants whose names did not appear in the 2002 voter list must fill out the left-hand column or the specific section based on the data of a relative (Father, Mother, Grandfather, or Grandmother) who was present in the 2002 list.

  • Name of Relative: Write the name of the relative whose data you are using (e.g., Father).
  • EPIC Number (2002): Copy the EPIC number exactly as it appeared in the 2002 list.
  • Relation: Write the relation exactly as mentioned in the ‘Relation’ column of the 2002 list next to that relative’s name (e.g., Father/Husband). Do not calculate the relationship anew; simply copy it.
  • Address & Assembly: Fill in the District, Assembly Constituency name, Part number, and Serial number exactly as per the 2002 list. Even if names have changed (e.g., Bardhaman splitting), use the old name recorded in 2002.

Situation 2: Name WAS in the 2002 Voter List

Applicants who are older and had their names in the voter list in or before 2002 (likely at their father’s house before marriage) must fill out the right-hand column or the section for their own information.

  • Elector’s Name (2002): Write the name and surname you had in 2002 before marriage (e.g., if you were Mukherjee before and Dutta now, write Mukherjee).
  • EPIC Number: Enter the EPIC number from that time.
  • Name of Relative: Write the name of the guardian listed in the 2002 list (usually Father). Even if there is a spelling mistake or different title in the old list, copy it exactly.
  • Address: Fill in the address, district, and assembly details exactly as found in the 2002 list.

Important Tips and Warnings

Please keep the following points in mind before submitting the form:

  1. Language: If filling out the form in Bengali, complete it entirely in Bengali. If in English, use Capital Letters throughout. However, it is better to write numbers in English.
  2. Exact Data: For 2002 information, always copy exactly what is in the old list, even if it does not match current data or contains spelling errors.
  3. Signature: The applicant must sign at the bottom of the form.
  4. Consult BLO: If using data for a Grandfather or Grandmother and the correct relationship option is not available on the form, clarify this with the BLO (Booth Level Officer) while submitting.

By keeping the 2002 voter list handy and following these steps, you can minimize the chances of errors while filling out the SIR form.
