SIR Form: Complete Guide to Filling the New Enumeration Form, Do not Make Mistake

SIR Form: The West Bengal Election Commission is set to begin a special Voter Enumeration phase for the final voter list of 2025. It is expected that this process might start from the 4th of November. While preparations are currently underway to initiate this process offline, the online procedure will also be launched soon. Every voter whose name is on the 2025 voter list must fill out this new form, and it is mandatory. In this report, we will discuss the complete step-by-step process for correctly filling out this form.

What Information Will Be Pre-Printed on the Form?

The form provided to you will have several details already printed on it, sourced from your current voter card database. You just need to verify if everything is correct. The pre-printed information includes:

  • Your name, EPIC No., and address.
  • The name and number of your Assembly and Lok Sabha constituency.
  • Part number, serial number, and the name of the state.
  • A special QR Code.
  • Your current photograph (which could be black-and-white or in colour).

What Information Do You Need to Fill? (2025 Data)

In the first section of the form, you need to update some of your recent information. Fill out this part with great care.

  • New Colour Photograph: You must affix a recent passport-sized colour photograph in the designated space.
  • Date of Birth: This is mandatory. It must be written in the specified format (DD/MM/YYYY).
  • Mobile Number: This is also mandatory. All future election-related information and OTPs will be sent to this number.
  • Aadhaar Number: This is optional. you can provide it if you wish, or you can leave this field blank.
  • Father/Mother/Spouse’s Details: You need to write the name of your father or guardian. If they have an EPIC number, you can mention it, but it’s not compulsory. Married women should provide their husband’s name and his EPIC number (optional). For unmarried individuals, write ‘Not Applicable’ in the ‘Spouse’s Name’ field.

2002 Information: Who Needs to Fill What?

The most crucial part of the form is providing the 2002 data. There are two separate sections here, depending on your year of birth.

1. For Those Who Were on the 2002 Voter List:
If you were born in or before 1984, your name is likely to have been on the 2002 voter list. In that case, you need to fill out the left-hand side of the form. Here, you will provide your own details from 2002, such as—your name at that time, EPIC number (if available), relative’s name (usually father), and the Assembly, Part Number, and Serial Number as per your 2002 address.

Special Note: Due to the reorganisation of districts, the name or number of the assembly constituency from 2002 might be different from the current one. There is no need to worry; just write the information as it was in the 2002 list.

2. For Those Who Were NOT on the 2002 Voter List:
If your birth year is after 1984, your name would not have been on the 2002 voter list. In this scenario, you do not need to fill the left-hand section. Instead, you must fill out the right-hand section of the form.

In this section, you need to provide the details of a close relative from your family whose name was on the 2002 voter list. You can provide the information of your father, mother, uncle, aunt, or any other close relative. You will need to mention that relative’s name, their 2002 EPIC number (if available), your relationship with them, and their 2002 voter list address (Assembly, Part Number, Serial Number).

After filling out the form, do not forget to sign in the designated space. If a family member is living elsewhere, you can fill out their form and sign it yourself, mentioning your relationship (e.g., ‘Father’ or ‘Mother’) in the ‘Relationship’ column. The final voter list for 2025 will be prepared based on this form, so fill it out with utmost caution.
